Forms & Application


Evaluations are conducted prior to a season beginning.  Here is the current Spring & Summer Evaluation Schedule for Boys Teams (link).  The evaluation form can be completed in advance and brought to the Evaluation (link).


New players who are invited to join a team must have their parent or legal guardian complete the membership application in the Club's Team Snap.  The application is available for online completion only and the link will be sent to you by your team's coach or team parent.  All membership information is routed to the Vipers Team Snap administrator for processing and filed by the Vipers club management.  Previous season players & parents must complete the membership application annually in September, and secure a new AAU Membership annually.  Contact your team’s coach or team parent for additional assistance.

  1. Make sure you've complete the online Membership & Waivers Form for your child.
  2. Provide your Team Parent with a copy of your child’s Birth Certificate or Passport.
  3. Apply for your National AAU Membership as an Athlete with extended benefits.  Alameda Vipers require all Players, Coaches, Assistant Coaches, Team Parents, Board Members and Area Coordinators to have a valid AAU membership.  The Vipers Club code for AAU is "WWEWBF" and go to the AAU Website to pay for and obtain your player's AAU membership.  Print out the membership card and give it to the Team Parent.

Coaching & Volunteers

Vipers team Coaches, Assistant Coaches and Team Parents complete the Volunteer Application upon joining the Vipers and then annually there after by Sept 1st. In addition to securing a new AAU Membership card, all non-player club members will go through a background check as a part of the AAU Membership process.  The application can be completed on-line with the linked provided by the Division Coordinator or via download and completion below.  Submit the forms to the Area Coordinator (see Contacts page).  The Vipers Club code for AAU is "WWEWBF" and go to the AAU Website to pay for and obtain your non-player Extended Benefits AAU membership. Email or print out the membership card and give it to the Team Parent.
